- Accounts email address
- Name of a contact in accounts
- Purchase Order No. (if required)
Participant details:
- First name of participant
- Middle name of participant
- Last name of participant
- Address
- DOB
- Tel number
- Email address
- Name of course
- Date of course
- City of course
Once we receive this information, we will book your participant/s in and send you an invoice.
